Health Systems

Health systems comprise the full range of organisations, people and actions whose primary intent is to promote, restore or maintain health. They function as complex adaptive systems, characterised by interdependence among financing, workforce, service delivery, information systems and governance. Global demographic shifts, rising chronic disease burdens and the demands of universal health coverage (UHC) have exposed persistent challenges: uneven access, workforce shortages, inefficiencies in care delivery, inequitable financing and gaps in quality and safety. The recent COVID-19 pandemic further underscored the need for resilient, integrated systems that can absorb shocks while maintaining essential services. Digital transformation, including telehealth and data analytics, offers opportunities to enhance equity, efficiency and patient-centred care. Sustained progress towards UHC and improved population health will depend on strengthening primary care, reforming payment models, optimising workforce planning, fostering interoperability of health information and ensuring transparent, accountable governance.

Large language models have been adapted to pharmacy practice to reduce prescription-direction errors in online pharmacies. By fine-tuning with expert-annotated data and embedding safety guardrails, an AI-driven “medication direction copilot” cut near-miss events by a third compared with existing benchmarks, demonstrating how advanced natural-language processing can bolster medication safety at scale. A pragmatic, cluster-randomised trial tested a machine-learning-based early warning system that mines real-time nursing documentation to detect patient deterioration. Deployment across multiple hospital units halved in-hospital mortality risk, shortened length of stay by over 10% and reduced sepsis events, illustrating the potential of real-time analytics to drive system-level gains in patient outcomes and resource use. An anatomical study of pulmonary arteries using computed tomography in heart-failure patients informed optimal placement of implantable pressure sensors for remote haemodynamic monitoring. By characterising vessel dimensions, depth and angulation, this work supports personalised device selection and placement, which underpins early detection of decompensation and may reduce readmissions through timely outpatient interventions.

Computerised provider order entry (CPOE): An electronic system for entering and managing medication and test orders to reduce handwriting and transcription errors. Near-miss event: An error intercepted before it reaches the patient, providing a safety signal without causing harm. Early warning system (EWS): A tool or algorithm that identifies patients at risk of clinical deterioration to trigger timely interventions. Universal health coverage (UHC): A financing and delivery goal in which all individuals have access to the health services they need without financial hardship.
