Summary
Physical activity represents a cornerstone of health promotion, with robust evidence linking regular movement to reduced risk of non-communicable diseases, improved mental well-being and enhanced longevity. Both structured exercise and incidental activity contribute to cardiovascular fitness, metabolic regulation and musculoskeletal integrity. A clear dose–response relationship emerges across populations: even modest increases in activity yield meaningful declines in all-cause mortality, while higher volumes of moderate-to-vigorous physical activity preserve functional capacity in later life. Interactions with genetic background, psychosocial factors and environmental context modulate individual responses, yet universal guidelines advocate at least 150 minutes weekly of moderate-intensity or 75 minutes of vigorous-intensity activity. Beyond physical health, regular engagement in sport and active recreation fosters social cohesion, cognitive resilience and workplace productivity, underscoring global significance and wide-ranging applications.

Recent analyses employing genetically informative designs have refined understanding of the relationship between habitual activity and mortality risk. One study using both animal models and human twin pairs demonstrated that shared genetic factors influence leisure-time activity, cardiorespiratory fitness and survival, suggesting that observed associations may partly arise from genetic pleiotropy rather than a simple causal pathway. Complementing this, a long-term twin cohort followed participants from mid-life into old age, examining trajectories of self-reported activity alongside other health habits. While baseline activity predicted later life moderate-to-vigorous physical activity, quantitative trait modelling indicated that over 80 % of this continuity was attributable to common genetic influences. Together, these findings emphasise the interplay between innate predisposition and behavioural factors in shaping health outcomes.

Leisure-time physical activity: Voluntary exercise or movement undertaken during non-working hours, such as sports, walking or cycling for recreation.
Epigenetic clock: A biomarker of biological ageing based on DNA methylation patterns that correlate with chronological age and predict morbidity and mortality.
Genetic pleiotropy: A phenomenon in which a single genetic variant influences multiple distinct traits, such as physical activity level and disease risk.
Dose–response relationship: The correlation between the magnitude of exposure (here, amount of activity) and the extent of health benefit or risk reduction.
Moderate-to-vigorous physical activity (MVPA): Activity that elevates heart rate and breathing, typically defined as 3–6 metabolic equivalents for moderate intensity and above 6 for vigorous intensity.
