Summary

Physical activity exerts multifaceted influences on mental health through neurobiological, psychosocial and behavioural pathways. Regular engagement in structured exercise or habitual movement elevates concentrations of neurotrophic factors, modulates neurotransmitter systems and attenuates systemic inflammation, collectively fostering resilience against depressive and anxiety disorders. Concurrently, participation in group-based or leisure activities enhances social connectedness, self-efficacy and stress-coping skills. Importantly, the relationship is bidirectional: poor mental health can undermine motivation and capacity for activity, while sedentary lifestyles may precipitate mood disturbances. Across age groups and cultural settings, tailored activity prescriptions have been shown to yield improvements in mood, cognitive function and overall life satisfaction, underscoring the potential for scalable public-health interventions that integrate physical activity into routine mental-health care.

One large-scale analysis of over 700 000 participants examined how varying patterns of leisure-time physical activity correlate with self-reported mental health burden across diverse age groups in China. Researchers quantified activity in metabolic equivalents of task (MET-min/week), frequency and intensity, revealing a markedly nonlinear, L-shaped dose–response relationship whereby the greatest reduction in mental health burden occurred up to approximately 1 200 MET-min/week. Distinct age-group analyses indicated that moderate swimming and apparatus exercise yielded particularly pronounced benefits for younger and older adults respectively. These findings highlight the importance of population-specific activity guidelines and support policy initiatives that encourage even modest increases in leisure activity to mitigate mental health burden.

Technical terms

Metabolic equivalent of task (MET): A unit that quantifies energy expenditure of physical activities, defined as the ratio of work metabolic rate to resting metabolic rate.

Dose–response relationship: A pattern describing how changes in the level of exposure (e.g., amount of physical activity) relate to changes in effect (e.g., mental health burden).

Mendelian randomisation: An analytical approach using genetic variants as proxies for modifiable exposures to infer causal relationships in observational data.

Leisure-time physical activity (LTPA): Any voluntary movement undertaken recreationally, distinct from occupational or transport-related activity, typically reported in structured domains such as sports, walking or swimming.
