Summary

Youth physical activity and sedentary behaviour patterns have been a focal point of public health research due to their profound influence on physical, mental and metabolic health across the life course. Globally, participation in moderate-to-vigorous-intensity physical activity (MVPA) declines sharply from early childhood through adolescence, while sedentary time—characterised by prolonged periods of sitting or low-energy postures—escalates. These shifts are observed across diverse settings and are influenced by sex, socioeconomic context and the built environment. Boys generally engage in more MVPA and accumulate less sedentary time than girls. Geographic comparisons reveal striking regional disparities, with lower activity levels in urban and high-income settings. Measurement advances, particularly the use of accelerometry, have refined our understanding of behaviour patterns and highlighted critical windows for intervention. Effective policy and environmental strategies are needed to reverse declines in activity, curb sedentary behaviour and support lifelong health among young people.

Evidence from recent policy analyses in a major East Asian context shows that despite the introduction of comprehensive national sports engagement programmes, overall physical activity grades among children and adolescents remain low, with certain indicators exhibiting paradoxical declines even as infrastructure expands. This research emphasises the need for integrated leadership across schools, families and communities to address systemic barriers and to elevate participation levels.

Investigations into the school environment have identified specific features that promote higher MVPA and adherence to activity guidelines. Active school policies, dedicated supervision committees, regular inter-school competitions, extended break durations and accessible outdoor facilities each contribute to increases in moderate-to-vigorous activity. The presence of crossing guards further augments daily activity levels, suggesting that relatively modest environmental and procedural changes can yield meaningful gains in youth physical activity.

Technical terms

Moderate-to-vigorous-intensity physical activity (MVPA): Activity requiring sustained energy expenditure above resting levels, such as brisk walking, running, cycling or organised sports.

Sedentary behaviour: Waking activities characterised by low energy expenditure while sitting, reclining or lying, including screen viewing and seated transport.

Accelerometer: Wearable motion sensor that records acceleration to quantify the frequency, intensity and duration of physical activity and sedentary time.

Adiposity: The amount of body fat tissue, often assessed by indices such as body mass index or dual-energy X-ray absorptiometry-derived fat mass index.
