Summary

Physical activity encompasses any skeletal‐muscle movement that raises energy expenditure above resting levels. Assessment approaches span self-report tools—such as questionnaires and activity diaries—to objective monitoring via accelerometers, heart-rate sensors and GPS devices. Precise characterisation of intensity, duration, frequency and context is essential to define dose–response relationships with health outcomes. Consistent evidence shows that greater volumes of moderate-to-vigorous physical activity (MVPA) and increased light-intensity movement are linked to lower adiposity, improved cardiometabolic profiles, enhanced mental wellbeing and reduced all-cause mortality. Moreover, breaking up prolonged sedentary behaviour yields independent benefits. Standardised metrics now inform global guidelines and population surveillance, guiding interventions that address physical inactivity across age groups, socioeconomic strata and geographic regions.

Recent advances have combined accelerometer data with dual-energy X-ray absorptiometry to untangle how different movement intensities influence body composition over time. In a large birth cohort followed from 11 to 24 years of age, each minute of light-intensity physical activity per day corresponded to a greater reduction in fat mass than an equivalent minute of moderate-to-vigorous activity, while each minute of sedentary time predicted incremental fat‐mass gains. Persisting in at least 60 minutes daily MVPA amplified fat-mass reductions, partly mediated by improvements in insulin sensitivity and lipid profiles. These insights highlight the potential of light-intensity activity as a strategic target in obesity prevention, especially among youth and those unable to engage in high-intensity exercise.

Accelerometry: The use of wearable sensors to record and quantify bodily movement in multiple planes over time.

Moderate-to-vigorous physical activity (MVPA): Activity that requires ≥3 metabolic equivalents, typically causing noticeable increases in heart rate and breathing.

Metabolic equivalent of task (MET): A unit expressing the ratio of work metabolic rate to resting metabolic rate, used to classify activity intensity.

Sedentary behaviour: Any waking behaviour characterised by low energy expenditure (≤1.5 METs) while sitting or reclining.

Dual-energy X-ray absorptiometry (DEXA): An imaging technique that measures body composition, including fat mass and lean mass, with high precision.
