Summary

Physical activity epidemiology examines the patterns, determinants and health consequences of movement behaviours across populations. Insufficient physical activity is a leading modifiable risk factor for non-communicable diseases such as cardiovascular disease, type 2 diabetes and certain cancers, as well as for mental health disorders and functional decline. Surveillance studies indicate that global prevalence of adults failing to meet recommended activity thresholds (at least 150 minutes of moderate-intensity or 75 minutes of vigorous-intensity activity per week) has risen over the past two decades, with persistent inequalities by sex, age, socioeconomic status and region. Analyses of activity domains reveal that work and household tasks remain the predominant source of moderate-to-vigorous physical activity (MVPA) in many low- and middle-income countries, whereas leisure-time activity contributes most in higher-income settings. Sedentary behaviour has emerged as an independent risk factor, with prolonged sitting linked to metabolic dysfunction and premature mortality. Growing evidence supports the effectiveness of multi-sectoral interventions—from urban design that promotes active transport through to tailored mass-media campaigns—in increasing activity levels and narrowing disparities. The global imperative is to integrate surveillance, policy and community engagement to reverse inactivity trends and achieve international targets for physical activity improvement by 2030.

Epidemiology: The study of the distribution, determinants and control of health phenomena in populations.

Moderate-to-vigorous physical activity (MVPA): Physical activity of at least moderate intensity that raises heart rate and breathing substantially, typically measured in minutes per week.

Physical activity domains: Contexts in which activity occurs, commonly classified as occupational/household, transport or leisure.

Sedentary behaviour: Any waking behaviour characterised by low energy expenditure while sitting or reclining.
