Physical Activity Impacts on Cardiovascular Health
Summary
Physical activity exerts a profound influence on the structure and function of the cardiovascular system, mediating risk factors across the lifespan. Regular movement stimulates endothelial function, enhances myocardial perfusion and promotes favourable blood‐lipid profiles. At the same time it attenuates systemic inflammation and improves autonomic balance. Benefits extend from reductions in resting blood pressure and arterial stiffness to improvements in cardiac output and ventricular remodelling. Both intensity and timing of activity appear to shape health gains, with evidence that moderate‐to‐vigorous exercise confers the greatest reduction in cardiovascular events, while even light activity offers protection beyond sedentary behaviour alone. These adaptations translate into lower incidence of coronary heart disease, stroke and heart failure, and contribute to declines in all‐cause and cardiovascular mortality worldwide. Interventions that incorporate varied intensities, break prolonged sitting and encourage consistent daily movement hold promise for broad public health impact.

Recent cohort analysis using continuous accelerometer data in over 92 000 adults reveals that timing of moderate‐to‐vigorous physical activity (MVPA) influences cardiovascular outcomes. While MVPA at any time of day was linked to lower risk of cardiovascular mortality, participants accumulating most activity in the middle of the day to mid‐afternoon experienced the greatest mortality benefit compared with those whose activity was concentrated in the morning or evening. These findings emphasise that not only volume and intensity but also temporal patterns of activity may optimise cardiovascular protection, particularly among older adults and those with pre‐existing heart disease.

Technical terms
Moderate-to-vigorous physical activity (MVPA): Any bodily movement requiring energy expenditure equivalent to brisk walking or more, generally ≥3 metabolic equivalents.
Light-intensity physical activity (LPA): Movement that expends energy at a level just above resting, such as slow walking or household tasks, typically 1.5–3 metabolic equivalents.
Sedentary behaviour: Waking activities characterised by very low energy expenditure while sitting or reclining, excluding sleep.
Accelerometer: A wearable device that objectively records movement and intensity over time, enabling quantification of activity patterns.
