Summary
Physical activity interventions have emerged as a cornerstone in the preservation and enhancement of cognitive health among older adults. These interventions span aerobic exercise, resistance and balance training, mind–body practices and technologically mediated programmes such as exergaming. Mechanistically, physical activity is understood to promote neuroplasticity, increase cerebral blood flow, up-regulate neurotrophic factors and mitigate neuroinflammation. As a result, improvements have been documented in global cognitive function, memory consolidation, executive control, attention and psychomotor speed. Programmes range from community-based group classes and home-exercise regimens to high-intensity functional training in residential care and virtual reality-augmented sessions. Key determinants of effectiveness include intervention intensity, duration and adherence, together with personalised progression and social support. Globally, such programmes have significant public health implications: they are cost-effective in preventing or delaying cognitive decline, reduce healthcare burden and support maintenance of independence. Translation into practice requires standardisation of protocols, robust training of instructors and integration with digital monitoring tools to ensure long-term engagement and measurable outcomes.
