Summary
Physical activity underpins health promotion and disease prevention worldwide. Measurement approaches range from self-report questionnaires to objective monitoring using accelerometers and heart rate sensors. Standardised units such as metabolic equivalents (METs) and step cadence allow comparison across studies and inform public health guidelines, which typically recommend at least 150 minutes of moderate-intensity activity per week in bouts of ten minutes or more. Recent emphasis has shifted towards relative intensity—expressed as a percentage of VO₂ reserve or heart rate reserve—to personalise prescriptions according to individual fitness, age and anthropometry. Advances in wearable technology and analytical methods now permit minute-by-minute characterisation of activity volume, intensity and patterns, enabling both clinical evaluation and large-scale surveillance. Challenges remain in harmonising devices, calibrating thresholds for diverse populations and integrating multidimensional metrics into clear recommendations for practitioners and the general public.

Step Cadence: The number of steps taken per minute, serving as a proxy for ambulatory intensity.
Relative Intensity: The exertion level expressed as a percentage of an individual’s capacity, for example VO₂ reserve or heart rate reserve.
Accelerometry: The use of motion sensors to capture bodily movement across one or more axes, enabling objective estimation of activity volume and intensity.
Wearable Sensor Metrics: Composite parameters derived from devices—such as step rate, heart rate patterns and activity bouts—that reflect both physical behaviour and physiological response.
