A large cohort study leveraged commercial wearable data linked to electronic health records to examine step volume and incident disease across the phenome. Participants walked a median of approximately 7 700 steps per day over four years, revealing inverse linear associations with obesity, sleep apnoea, reflux disease and depression, with protective effects plateauing above roughly 8 200 steps. Nonlinear relations emerged for incident diabetes and hypertension, indicating minimal additional risk reduction beyond 8 000–9 000 daily steps. This work provides real-world evidence for clinical guidance, suggesting that maintaining step counts above identified thresholds may help reduce the burden of chronic disease.
