Summary

Postural control in individuals with low back pain encompasses the integrated function of sensory detection, central processing and motor responses required to maintain balance and orientation. Altered proprioceptive input from the lumbar spine, changes in muscle activation patterns and shifts in hip–spine coordination contribute to compensatory strategies that manifest as increased body sway, delayed corrective responses and variable joint‐angle perception. Chronic pain affects the weighting of visual, vestibular and somatosensory signals, leading to stiffer postural strategies or increased reliance on non‐painful segments. Acute episodes provoke further adaptations, with distinct clusters of coordination patterns emerging in both sagittal and frontal planes. Impaired sensorimotor integration and kinematic coherence between spinal segments and hips may compromise functional performance under perturbation, while demographic factors such as age, body mass index and disability level interact to exacerbate instability. A thorough understanding of these mechanisms has global significance for designing targeted rehabilitation programmes, refining clinical assessment tools and reducing fall risk through interventions that restore proprioceptive accuracy and optimise motor control strategies.

A comprehensive meta‐analysis of postural sway during quiet standing established that individuals with chronic low back pain exhibit greater centre of pressure excursions than healthy controls across vision and surface conditions. Effect sizes were moderate with eyes open and increased markedly when vision was removed or support was unstable. The analysis revealed that greater self‐reported pain intensity correlates with amplified sway, particularly under eyes‐closed, firm‐surface conditions. These findings underscore the importance of proprioceptive reweighting deficits and suggest that interventions should address sensory integration to mitigate instability.
