Summary

Traditional face-to-face therapies, such as cognitive behavioural therapy for psychosis, target the appraisals and responses to voices by challenging beliefs about voice omnipotence and control. Specialist approaches like avatar therapy enable a structured dialogue between the voice hearer and a digital representation of the voice, promoting mastery and reduction of distress. Compassion-focused therapy seeks to shift the individual’s motivational system from threat-based to affiliative patterns, enhancing self-regulation and reducing hostile interactions with voices. Mindfulness-based and acceptance-oriented interventions cultivate non-judgemental awareness of voices and facilitate decentring, lowering distress associated with negative content. Digital and mobile health tools, including smartphone applications and immersive virtual reality platforms, are proliferating, offering scalable and engaging means to deliver self-help modules, guided exercises and real-time monitoring. While these modalities hold promise for increasing accessibility, their optimal configuration—such as the role of therapist guidance, user involvement in design and integration with standard care—remains under investigation. Together, these strategies reflect a shift from symptom suppression towards relational and experiential change, acknowledging auditory hallucinations as complex cognitive-affective phenomena.
