Summary

Schizophrenia is increasingly recognised not only as an early‐onset psychiatric condition but also as one that alters the trajectory of ageing across psychological and social dimensions. Individuals with schizophrenia commonly experience accelerated cognitive decline, diminished social engagement and increased vulnerability to age‐related physical illnesses. These changes interact with external factors such as stigma, social isolation and barriers to employment, producing a pattern of premature functional impairment and reduced quality of life in mid to later adulthood. Psychosocial ageing in schizophrenia thus refers to the combined influence of neurobiological alterations, psychological resilience and social context in shaping an individual’s adaptation over the lifespan.

Longitudinal and cross‐sectional research has documented deteriorations in social cognition, reductions in cognitive reserve and heightened loneliness among older patients with schizophrenia. Community‐based psychosocial interventions that focus on skills training, peer support and vocational rehabilitation have shown potential to slow decline and bolster daily functioning. Nonetheless, outcomes remain heterogeneous, reflecting variations in illness course, treatment access and socio‐economic conditions across different settings.

Cognitive reserve: The brain’s resilience to neuropathological damage, influenced by education, occupational complexity and engagement in mentally stimulating activities.

Psychosocial ageing: The dynamic interplay of psychological well-being, social relationships and environmental influences that shapes functional ageing, particularly in chronic mental illness.

Social cognition: The mental processes involved in perceiving, interpreting and responding to social information, including emotion recognition and theory of mind.

First-episode schizophrenia: The initial phase following the emergence of psychotic symptoms, before or shortly after the start of antipsychotic treatment, critical for prognosis and targeted interventions.
