Respiratory Protection Fit Assessment in Healthcare Settings
Summary
Respiratory protection fit assessment ensures that healthcare workers are effectively shielded from airborne hazards such as pathogens and aerosols. A secure seal between facepiece and skin is critical to prevent inward leakage and guarantee the nominal performance of filtering facepiece respirators, including N95 and FFP2 types. Two principal methods—qualitative and quantitative fit testing—evaluate seal integrity by relying on sensory detection of test agents or by measuring particle concentration ratios inside and outside the mask. Fit performance depends on factors such as facial anthropometry, respirator design and material properties, user training and the dynamic demands of clinical tasks. Recent advances in three-dimensional facial scanning, virtual fit analysis and custom additive manufacturing enable bespoke mask designs, while novel sealing materials like viscoelastic foams enhance comfort and conformity. Regulatory bodies mandate periodic fit assessment to ensure compliance and to reduce healthcare-associated infections, emphasising the global importance of standardised, accessible fit-testing programmes.

Respiratory Protection Fit Assessment in Healthcare Settings publication trend
The graph below shows the total number of articles in respiratory protection fit assessment in healthcare settings across all publications each year (not limited to Nature Index journals).
Technical terms
Fit factor: A numerical ratio comparing particle concentration outside the respirator to that inside, used to quantify seal integrity.
Quantitative fit testing (QNFT): An objective method employing particle-counting instruments to assess the fit of a respirator.
Qualitative fit testing (QLFT): A subjective pass/fail method based on the wearer’s detection of a test agent to evaluate fit.
Respiratory protective equipment (RPE): Devices such as filtering facepiece respirators or powered air-purifying respirators designed to protect the wearer from inhaled hazards.
Anthropometry: The measurement of human body dimensions, applied to develop fit test panels and inform respirator design.
