Summary

Self-injurious behaviour (SIB) comprises a range of repetitive, deliberate actions that result in physical harm to one’s own body, including head-banging, self-biting and skin-picking. It is highly prevalent in individuals with autism spectrum disorder and those with intellectual disabilities, affecting up to 40–50% of some cohorts. The severity and persistence of SIB are influenced by a combination of biological, sensory and behavioural factors. Neurobiological pathways implicate altered pain processing and atypical sensory reactivity, while behavioural dysregulation characterised by impulsivity, overactivity and communication deficits often precedes onset and predicts chronicity. Understanding the typology and developmental trajectory of SIB is essential for designing effective early-intervention strategies. A global perspective emphasises that culturally sensitive assessment tools and multidisciplinary care models—spanning behavioural therapy, pharmacological support and, in some cases, specialised neurosurgical or neuromodulatory approaches—are critical to improving quality of life for affected individuals and their caregivers.
