Severe hypoglycaemia, defined by neuroglycopenic symptoms requiring external assistance, poses an immediate threat to cardiac function and long-term vascular health. Controlled induction of hypoglycaemia in people with type 2 diabetes revealed a delayed but sustained inflammatory and oxidative stress response. In comparison with matched controls, patients exhibited elevated concentrations of multiple cytokines and acute-phase proteins 24 hours after a one-hour hypoglycaemic clamp. High-sensitivity C-reactive protein levels remained significantly raised at 24 hours, and urinary isoprostane excretion increased, indicating ongoing lipid peroxidation. Proteomic profiling identified a suite of stress-related mediators that returned to baseline only after 24 hours. These findings provide molecular evidence that a single hypoglycaemic event can initiate prolonged vascular stress signals, underscoring the need for strategies that prevent even transient severe glucose falls.
