Social Cognition in Schizophrenia
Summary
Social cognition encompasses the mental operations underlying social interactions, including perception of others’ emotions, attribution of intentions and the capacity for self-reflection. In schizophrenia, deficits in social cognition are pervasive and multifaceted, manifesting as impaired emotion recognition, reduced theory of mind, altered self-referential processing and disrupted reward learning. Neurobiological studies have implicated dysconnectivity among prefrontal, cingulate and temporoparietal regions, often accompanied by neurotransmitter imbalances such as glutamatergic dysfunction. These impairments contribute to social withdrawal, vocational difficulties and diminished quality of life. Recent advances have focused on dissecting specific subprocesses—such as trust evaluation and context-dependent decision-making—and on integrating neuroimaging, electrophysiology and computational approaches. Understanding the neural and cognitive mechanisms of social dysfunction informs targeted interventions, from cognitive remediation and social skills training to pharmacological modulation and neuromodulation techniques. Given the global burden of schizophrenia and the universal importance of social functioning, elucidating these mechanisms holds promise for improving clinical outcomes and social reintegration worldwide.

Recent studies have combined neurochemical and functional imaging to probe the neural substrates of social decision-making in schizophrenia. In a trust-investment paradigm, participants with treatment-responsive and treatment-resistant profiles underwent proton magnetic resonance spectroscopy in the anterior cingulate cortex alongside functional MRI during social exchanges. Both patient groups exhibited reduced investments compared with controls, but treatment-resistant individuals showed distinct glutamate-linked signal changes in dorsolateral prefrontal and parietal association cortices. In treatment-responsive cases, alterations emerged in the anterior caudate. These findings delineate how glutamatergic variations modulate cortical and subcortical reward networks during interpersonal trust, offering potential diagnostic markers and pinpointing neurotransmitter targets for novel therapeutic approaches.
