Statin-Based Therapeutics in Periodontal Disease

Summary

Statins, widely prescribed for cholesterol reduction, have emerged as promising adjuncts in the management of periodontitis through their anti-inflammatory, antioxidant and bone-modulatory properties. Beyond lipid lowering, these agents inhibit key inflammatory pathways and promote osteoblastic activity, thereby attenuating alveolar bone resorption and supporting tissue regeneration. Both systemic and local applications of statins have been explored: systemic dosing may modulate the global host response, while local delivery vehicles—gels, prodrug conjugates or biodegradable carriers—ensure sustained drug concentrations at periodontal defects. Preclinical models demonstrate that statins reduce levels of proinflammatory cytokines, oxidative stress markers and matrix-degrading enzymes, while enhancing expression of bone-forming proteins. Clinical studies suggest that subgingival statin application as an adjunct to scaling and root planing can lead to greater probing depth reduction and attachment gain compared with mechanical therapy alone. Taken together, statin-based interventions represent a host modulation strategy with the potential to improve periodontal stability and long-term oral health outcomes.

A large-scale longitudinal study using national health-screening cohort data examined the relationship between statin use and the incidence of chronic periodontitis. Contrary to expectations of a protective effect, long-term statin users exhibited a modestly higher rate of periodontitis compared with non-users after adjustment for confounders. Subgroup analyses showed this association to be consistent across age and sex strata, suggesting that prolonged systemic exposure to statins may influence oral microbiome composition or immune surveillance in ways not fully understood. These findings highlight the complexity of extrapolating systemic drug benefits to periodontal tissues and underscore the need for mechanistic studies to determine whether observed associations reflect causal effects, confounding by indication or variations in healthcare access and diagnostic patterns.

Alveolar bone loss: The pathological resorption of the jawbone surrounding tooth roots, a hallmark of periodontitis leading to tooth mobility and loss.

Host modulation therapy: Treatment strategies aiming to alter the patient’s immune and inflammatory response to prevent tissue breakdown and promote repair.

Cytokines: Small proteins released by cells that mediate and regulate immunity, inflammation and tissue healing processes.
