Subjective and Objective Assessment of Sleep Quality
Summary
Sleep quality is evaluated through two complementary lenses: subjective reports and objective measurements. Subjective assessment relies on self‐reported tools such as questionnaires and sleep diaries, capturing personal perceptions of restfulness, ease of falling asleep and perceived interruptions. Objective assessment employs technologies including polysomnography, electroencephalography and actigraphy to record physiological signals, sleep stages and movement. Discrepancies between these approaches are common, reflecting the multifaceted nature of sleep experience, influenced by mood, environment and individual expectations. Integrating both perspectives offers a richer understanding of sleep health, guides diagnosis of disorders and informs personalised interventions. With wearable devices and mobile EEG systems gaining traction, the field is moving towards real‐time monitoring and tailored feedback to improve sleep hygiene and clinical outcomes globally.

Recent evidence demonstrates that fluctuations in objectively measured sleep architecture correspond with variations in individuals’ subjective sleep ratings. Within‐subject analyses reveal that shorter sleep onset latency, reduced wake after sleep onset and higher sleep efficiency lead to better self‐reported sleep quality, overturning earlier notions that subjective reports are unrelated to physiology. Detailed mobile EEG studies corroborate that quantitative changes in sleep microstructure reliably influence morning sleep impressions.
Foundational work has also probed the link between subjective sleep quality and cognitive performance. Large‐scale assessments combining multiple self‐report scales found no consistent association between perceived sleep quality and executive functions, working memory or procedural learning in healthy young adults. This suggests that while subjective sleep ratings reflect internal experience, they may not predict next‐day cognitive outcomes in non‐clinical populations.

Subjective sleep quality: An individual’s self‐report of restfulness, continuity and satisfaction with sleep.
Objective sleep quality: Quantitative measures of sleep stages, arousals and movement derived from physiological recordings.
Polysomnography: A comprehensive overnight recording of brain waves, eye movements, muscle activity and cardiorespiratory variables.
Actigraphy: Monitoring of gross motor activity via a wrist‐worn device to infer sleep–wake patterns over extended periods.
Sleep onset latency (SOL): The duration between lights out and the onset of sleep.
Wake after sleep onset (WASO): Total time spent awake after initial sleep onset until final awakening.
Sleep efficiency (SE): Ratio of total sleep time to time in bed, expressed as a percentage.
