Summary
Three-dimensional echocardiography (3DE) has revolutionised the assessment and management of congenital heart disease by offering volumetric reconstructions of cardiac anatomy in real time. Unlike two-dimensional imaging, 3DE captures entire chambers, valves and septal structures in a single data set, allowing precise quantification of volumes, dynamic visualisation of intracardiac flow and detailed appraisal of complex lesions. This modality has become integral to pre-operative planning, enabling surgeons to appreciate the spatial relationships of defects and to simulate surgical repairs or device closures. In the catheter laboratory, live three-dimensional guidance enhances the safety and efficacy of interventional procedures, from atrial septal defect closures to valve implantations.
Technological advances in matrix-array transducers, multiplanar reconstruction and high-resolution rendering have expanded clinical applications. Colour flow and Doppler modalities in three dimensions permit calculation of regurgitant volumes and shunt quantification, while fusion imaging combines 3DE with fluoroscopy or computed tomography for comprehensive procedural guidance. Emerging post-processing techniques, such as TrueVue light source simulation and glass-mode rendering, further improve tissue contrast and transparency, facilitating interpretation by multidisciplinary teams. TrueVue imaging: A proprietary rendering technique that simulates a virtual light source to enhance depth perception and tissue contrast in three-dimensional echocardiography.
